Digital Lockout / Tagout for Malaysia

Built for Malaysia's amended OSH Act.

Enforces OSHA 1994 compliance physically, with full audit traceability, not just paperwork. A partnership between Symple and LOTOBOX, built for Malaysia's process and manufacturing sites.

The problem

Analog LOTO relies entirely on human vigilance.

Unsafe tool access before isolation

Technicians can reach live equipment before it's actually isolated, a direct path to serious injury.

Bypassed steps, no full record

Paper checklists rely on memory. When a step is skipped, there's no end-to-end record to catch it.

Uncontrolled key access

Lost or duplicated keys mean anyone can open what should be locked, undermining the point of lockout.

At a frozen chicken processing factory near Batu Pahat, Johor, a worker cleaning a spin chiller machine fell into it and was crushed by the screw conveyor. DOSH issued a prohibition notice, citing the employer's failure to provide a safe work system, especially a lockout/tagout system for machinery cleaning work. (DOSH Johor, 2021)

WHAT MAKES THIS DIFFERENT

What if the system enforced it, instead of reminding people to?

This system enforces compliance physically, not just records it.

The wrong key simply cannot be removed from the cabinet.

The solution

Three components.
One enforced procedure.

01 / SOFTWARE

Symple

Issues LOTO-based work orders, manages permits, and logs every step, consistent, traceable, and audit-ready from start to finish.

02 / HARDWARE

LOTOBOX

A secure, connected cabinet that locks each padlock and key in a fixed position. Technicians can only remove what they're authorised for.

03 / YOUR PROCESS

Your LOTO Procedure

Defines each work order and controls which keys release at every step, fully integrated, and it scales from one site to hundreds.

The mechanism

Proof it's enforced, not just tracked.

Wrong key stays locked

Everything except the technician's authorised key remains physically locked, no override.

Green light confirms return

Returning a key to the correct slot triggers a real-time green light, visible, verifiable confirmation.

New work order required

Once a job closes, the cabinet re-locks itself. Reopening it requires a fresh, authorised work order.

How it works

Every step, enforced in order.

LOTOBOX's 12-action breakdown of OSHA 29 CFR 1910.147, the 6-step Control of Hazardous Energy standard, run two different ways. This is an example of how LOTOBOX can be applied to strengthen lockout/tagout enforcement.

Before Work Starts

Preparation & access control

  • Work order issued in Symple; keys auto-allocated
  • Access authorised in the LOTOBOX
  • Technician retrieves the correct padlock(s) from the LOTOBOX
  • Equipment fully isolated; padlock key(s) returned
  • Permit to work issued
02

During Work

Isolation enforcement

  • Work proceeds under the active permit
  • Every action recorded automatically in Symple
03

After Completion

Deisolation & closeout

  • Padlock key(s) collected from LOTOBOX and padlock(s) unlocked and removed except the padlock for power isolation
  • Supervisor unlocks and removes padlock for power isolation
  • Supervisor performs safety inspection
  • Equipment test-run and verified safe
  • Padlock(s) and key(s) returned to the LOTOBOX
  • Permit closed; work order signed off
